Please enter your credit card payment information below. Upon your acceptance into this program, your bank account will be charged a Monthly Coaching Fee of $175 per month from this current month through December 2024. Unless otherwise cancelled within 30 days of the completion date, your membership will auto-renew for a Monthly Coaching Fee of $175 per month. By including your signature below, you authorize The Swag Coach® Program to charge your credit card in accordance with Participation in this Program. IF your application is not approved, your credit card will NOT be charged.

PROGRAM DETAILS & RULES OF ENGAGEMENT
✔️ I understand that as a participant of The Swag Coach® Small Group Coaching Program, I am committing to attend once per month group coaching calls, excluding Holidays, as scheduled with my coach, with the intent to apply my learning to my business to learn and grow.

✔️ I understand that I am applying to be a participant in The Swag Coach® Small Group Coaching Program for the remaining months of 2024, which will automatically renew on an annual basis unless cancelled with at least 30 days written notice.

✔️ I understand that every participant in The Swag Coach® Small Group Coaching Program is a promotional product sales rep or small business owner, and that their time is valuable. I am dedicated to showing respect and appreciation for their time, effort, and commitment for helping me during our small group coaching sessions, and in return, commit to preparing for, and engaging in, these small group coaching sessions. I agree to a fee of $25 per missed SGC coaching call.
✔️ Confidentiality: I agree that everything that is said in small group meetings is considered strictly confidential and stays within the group unless otherwise allowed. I agree to only utilize recorded coaching sessions for myself and share with my group.
✔️ I understand there is a STRICT no stealing accounts policy and no recruiting policy. The spirit of this program is to create a safe environment. If another participant is asking me for account help, I can assist but otherwise agree to not contact ANY accounts discussed by another participant.
✔️ I agree that during the term of this agreement, and for a period of twelve months afterwards, I will not directly or indirectly hire, or attempt to hire for employment or as an independent contractor any person who has been a Swag Coach® affiliate, Small Group Coaching member or independent contractor at any time. If I violate this provision, I agree to pay The Swag Coach® a recruiting and placement fee of fifty thousand dollars ($50,000) per affiliate, member or independent contractor solicited or hired.
✔️ I understand the fees I am paying are for me personally, and not for other individuals in my company or network. I cannot bring guests nor have a substitute sit in for me during any coaching session, unless otherwise authorized by The Swag Coach® Program.
✔️ I understand I will be placed into a Small Group as assigned by The Swag Coach® Small Group Coaching Program, and I agree to that placement made on my behalf.

✔️ I understand this is a commitment through Dec 31, 2024 and if I discontinue participation in The Swag Coach® Small Group Coaching Program prior to the completion of this timeframe, the unpaid balance will remain due in full to The Swag Coach Program. I agree upon notice of cancellation to charge this remaining balance due, in full, to my credit card on file.

✔️ I understand The Swag Coach® Small Group Coaching Program reserves the right to revoke my membership and my participation, at its sole discretion, without refund.
